It is the sentence I hear more than any other. A woman describes months of broken sleep, weight that won't shift, a temper she doesn't recognise and a brain that keeps losing words — and then adds, almost apologetically, “but my blood tests came back normal.”
She says it as though it settles the matter. It doesn't.
Why hormone levels are a poor snapshot in perimenopause
The defining feature of perimenopause is not that hormones are low. It is that they are erratic.
Oestrogen in particular can swing high and low, sometimes within the same cycle, sometimes reaching levels higher than in your thirties before dropping sharply. Progesterone tends to fall earlier and more consistently as ovulation becomes less reliable. FSH rises — but it also fluctuates, which is exactly why a single reading is unreliable.
So a blood test taken on a Tuesday tells you what your hormones were doing on Tuesday. It does not tell you what they did last week, and it certainly does not tell you what your body has been coping with for eighteen months.
Hormone levels fluctuate considerably during the menopausal transition, so a single blood test may not tell the whole story. Your symptoms, your history and your overall health matter too.
“Normal” is a range, not a verdict
A reference range describes where most results in a population fall. It does not describe where your body functions well. Two women can have identical results and entirely different experiences, because symptoms are driven by the rate and size of hormonal change as much as the absolute number.
This is why perimenopause is usually a clinical diagnosis in women over 45 — based on symptoms and cycle changes, not on a lab printout.
What is actually worth testing
Hormone testing has limited value for diagnosing perimenopause in the typical age group. But testing has real value for a different purpose: ruling out the conditions that produce identical symptoms and are entirely treatable. It is genuinely worth asking about:
- Thyroid function — hypothyroidism mimics almost every perimenopausal symptom, and is common in this age group
- Full blood count and ferritin — iron deficiency is very common in women with heavy perimenopausal bleeding, and produces fatigue and brain fog
- Vitamin D and B12 — both frequently low, both affect energy and mood
- HbA1c and lipids — metabolic risk shifts in midlife and is worth knowing regardless of symptoms
- FSH — genuinely useful if you are under 45, where earlier menopause needs to be identified rather than assumed
Notice that most of this list isn't about sex hormones at all. That's the point. The useful question is not “are my hormones normal?” but “what else could be contributing, and has anyone actually looked?”
What to do with a normal result
A normal result is genuinely reassuring — it means several serious things have been excluded. What it does not mean is that nothing is happening, or that you should go home and try harder.
